I (including my minor child(ren), if applicable) hereby acknowledge that my participation in the Situational Awareness and Self-Defense Class (the “Class”), is completely voluntary and that I am able to cease participation in the Class at any point.

1. The Class may involve some physical activity and contact with the instructor or other class participants. I am fully aware that such physical activity and physical contact, or my participation therein, may be dangerous and involve risks and dangers of me falling ill, being seriously injured, and/or killed. In addition to physical injury, I am fully aware that any physical activity, physical contact, or related activities involve risks and dangers of financial injury and loss. In addition to the aforementioned risks, I am fully aware that any physical activity, physical contact, or related activities involve risks and dangers to my physical property including, but not limited to, eyeglasses, watches, and phones. I am further aware that such risks of physical injury, financial loss, and personal property damage exist with respect to any such physical activity. Despite such risks of injury, loss, and damage involved in any such physical activities, I freely and voluntarily choose to be involved, assuming all risks of illness, injury, or death.

2. Because I understand and assume all risks involved in participation in the Class, I hereby waive any and all legal or other claims that I, my assignees, heirs, guardians, legal representatives, and/or successors may have, if any, against Intentional Self-Defense LLC, it's employees or assignees and any and all people associated with this Class (a “Released Party”), for any and all injuries that I may incur, including my death, as a result of my participation in the Class. Such risk of injury that I am knowingly and voluntarily assuming, and for which I am releasing liability, may result from the negligence of the Released Party, my own negligence, or the negligence of any third party while I am participating in the Class. I acknowledge that this Release and Waiver of Liability is being relied upon by Intentional Self-Defense LLC in allowing me to participate in the Class and to be present in the facility in which the Class takes place.

3. I agree to indemnify, defend, and hold harmless the Released Party from and against any and all claims, demands, actions, and causes of action, including costs and attorney's fees, arising out of or related to my participation in the Class, whether caused by my negligence or the negligence of others, except to the extent caused by the willful or wanton misconduct of the Released Party, as provided under Minnesota law.

4. I understand that there is no guarantee that taking this Class will keep me safe. This is an introductory course. I acknowledge that I will not be an expert after taking the Class. Additional training under a qualified professional and years of practice are recommended. I acknowledge that the use of the principles and techniques taught in this Class is at my own risk. Being more aware of my situation does not mean I will not be injured, killed, or find myself in a dangerous or frightening situation. I acknowledge that the techniques are not applicable to all situations. I further acknowledge that the use of the techniques must be in accordance with the law, especially use-of-force laws. I acknowledge that the Released Party is not a lawyer, and is not providing legal advice. I acknowledge that I can obtain advice from a lawyer regarding the relevant laws that may apply relating to the use of any of the principles or concepts taught in the Class, including but not limited to civil or criminal laws relating to torts, crimes, self-defense, and property laws.

5. I understand that the Released Party may take photos or videos during sessions. I consent to the use of my name, voice, photograph, and/or likeness, in any manner, including but not limited to (1) on or in products, merchandise, goods, or services; (2) for purposes of advertising or selling; or (3) for soliciting purchases of products, merchandise, goods, or services. I further release the Released Party from any and all liability arising out of the use of my likeness, including any claims for invasion of privacy, defamation, or misrepresentation.

6. If any provision of this Agreement is held to be invalid, illegal, or unenforceable, the remaining provisions shall continue to be valid and enforceable to the fullest extent permitted by law.

7. This Agreement shall be governed by and construed in accordance with the laws of the State of Minnesota, without regard to its conflicts of law principles.
